Package Prep Form (for those choosing the Highlight Montage)
NOTE:
This form should only be filled out when all of your dates, times
and other pertinent information is finalized. It's a good idea not
to even send this until a week or two before your ceremony! That way,
we'll know everything is accurate and likewise, it won't get lost
in a computer crash or something of that nature.
This
information will help us get all of your details in order for the
day of your big event and for the actual editing of your video! Some
of this information may have been covered in our contract, but this
is what we really rely on. When you are finished answering your questions,
review the answers and click the submit button at the bottom of the
page. You will be emailed the results of your form and Matt will contact
you to let you know that we have everything we need! If a question
does not apply to you because we made a change to your video package
adding or subtracting an option, put N/A down for that particular
question as your answer. You'll have the opportunity at the bottom
of this form to explain the change and follow it up with any answers/information
we'll need to execute the production of your video.
First,
you'll need information on us. Use the phone number below to contact
us in case of emergency or on the day of your big event. Any other
questions should be directed by email. We'll call you if we have to!
The address is for you to send us pictures if you're having a montage
done or anything else that you may need to send us. You'll also have
to get us your remaining balance either sometime before your big day
or on the day of. That is expected before the ceremony begins, please
adhere to this request! Anyway, here's our info:

a city, state and zip code where we can send your video when it is

IMPORTANT: If your address is an apartment or PO
box, you should provide an address that can accept a large package
for obvious reasons... maybe a parent or friend's address. Likewise,
if you provide your shipping address, be sure somebody is collecting
your mail or you have put your mail put on hold with the post office.
In many cases, Matt finishes your video before you get back from your
honeymoon. We do not require the post office to get signatures for
videos alone (they are easily replaceable), but if you are providing
pictures for a montage we WILL require a signature for delivery:

Are there any special rules or parameters that we have to follow at
the ceremony location? If the information helps, we use two
camera's for all of our shoots and like to be up front on a knee leaning
around the first row of seats, if room allows, as the bride comes
down the aisle. Once she's just about up front, we move out of the
way and get shots from the sides with that camera. All the while a
main camera somewhere centrally located on the floor or balcony is
moved into position and recording the whole time. If you don't know
of any rules or parameters or if our standard approach sounds fine

This information will help Matt edit and produce your video in a timely
fashion so you can get it back, in most cases, when you get back from
your honeymoon! First we'll cover musical selections for your video.
In your package, you'll be getting a Video Introduction, Ceremony
Coverage including the following formal photo shoot, Reception coverage,
the Highlight Montage and the Spotlight Video! The next few selections
will indicate which parts of the video I will need song selections
for. Simply indicate the songs title and artists name that you would
like for each different part. If you want us to 'wing-it', simply
put N/A in the text field and you'll have the opportunity below to
give us an idea of what you would like to hear! |

You can customize your Highlight Montage to your liking. You
can choose to have the montage run as a 'day in rewind', that is starting
with stills from your reception and ending on your ceremony, or you
can have the montage run chronologically (starting with your ceremony
and ending on your reception). Likewise, you can choose your transitions
from still-frame to still-frame like simple cross dissolves, a combo
of cross dissolves and page peels, or an all out use of all sorts of
'jazzy' types of transitions. Make your selection here:

--You're
getting 4 DVD's of your event. You will want to keep one
DVD copy in a safe place, like a safety deposit box. As I archive
your video in three different ways, you never know if my studio
will get picked up by Godzilla one day and thrown clear out to sea.
In the extreme and unlikely event that something like that might
actually occur, rest assured you will have a duplicatable video
stashed away for safe keeping! Likewise, if you want more than 4
copies, additional copies are $35. You can
also add on your raw footage outakes now for the special price of
$100 (normally $150). This is the only time
I can offer the raw footage at this price because it will be on
my systems until your main video is completed. Once I archive your
main video, I lose the raw footage and the process would take much
longer to produce the raw footage once your project has been archived.
If you're interested in adding on your raw footage (you can do this
at any time before I send your completed video to you), email
me and ask for the 'secret link' to the special pricing page,
which will allow you to add the raw footage and any extra copies
of your video via a Paypal shopping cart. When you check out, I'll
get your order and will send your raw footage and any additional
videos with your completed video. You would get 4 copies of the
raw footage to compliment your main video disks.

For your Video Intro, I will use your wedding program for
any credits we put in your video including any 'In Memory Of' section
of the program. If your program is missing anything you would like
seen in the video, credit wise for those who have passed for example
if not already in the program, indicate that here along with those
names. Likewise, if there are any spelling errors in the program,
let us know ASAP.

This should cover everything. If you have any special
requests, indicate them here and I will address them in an
email when I get the results of this form for review. If not, leave
this section blank. If your ceremony is expected to last more than
30 minutes long, the total length of your video will be extended to
fit that extra time. The Basic Package estimated video time length
is based on a thirty minute ceremony. For example, if your ceremony
will be about 30 minutes long, your video will be around 1 hour 15
minutes long. If your ceremony will be an hour long, your video will
be 1 hour 45 minutes long. Hope that makes sense! Likewise, if you've
added other options to your package in our contract, use this area
to indicate that and to put down any music selections that might be
needed to execute the production of that package option in your video.
